Friday, August 29 is the last day to clear your packet so, what do you need to do?

1) Pick up in a sports packet in the student activities office

2) Get a Bookroom Clearance

3) complete the packet including a current physical (dated after July 1, 2014.

4) Complete the imPACT test -- go to  www.SignUpGenius.com/go/60B044AADA92EA20-impact  THURSDAY, AUGUST 28, 3:45 pm in P-13

5) Return your completed packet to the Student Activities Office

 

 

IMPORTANT for NEW ATHLETES!!!

ImPACT Tests for all new Athletes are:

August 28, 2014-3:45 pm--> P-13

You must sign up at www.SignUpGenius.com/go/60B044AADA92EA20-impact for available test slots.

All new athletes must ImPACT test before they can clear their athletic packet. The permission slip is required to take the test--Download it here! If you tested last year you do not need to retest this year.

TEST time is start time! Please arrive 15 minutes prior!

Volunteer Drivers

  To volunteer as a driver you must:

  • Complete the GUS #153  (Driver information form and DMV pull notice)
  • Read and Sign GUS #155 (Instructions for Transporting GUSD Students)
  • Provide a copy of the first page of drivers Automobile Insurance Policy Declaration
  • Provide a copy of the Driver’s License
  • Obtain and Submit a DMV H6

Return all of this to the GHS Secretary
