The imPACT date for new athletes is 1/27/15. Please sign up for the test at the link below.

Already played a sport this school year? Pick up a new parent consent form and return the completed form with a bookroom clearance in order to try-out for your new sport.

Volunteer Drivers

  To volunteer as a driver you must:

  • Complete the GUS #153  (Driver information form and DMV pull notice)
  • Read and Sign GUS #155 (Instructions for Transporting GUSD Students)
  • Provide a copy of the first page of drivers Automobile Insurance Policy Declaration
  • Provide a copy of the Driver’s License
  • Obtain and Submit a DMV H6

Return all of this to the GHS Secretary
