Sports Clearances for 2015-2016

Sports Packets are now available in the Student Activities Office!

Please pick up a packet to begin your Clearance Process! 

General Requirements to obtain a Sports Clearance at GHS:

  1. Complete the Online Clearance Process
  2. Return Parent Signature Form
  3. Return Emergency/Consent Form
  4. Return Completed/Stamped Physical Form
  5. Complete the ImPACT test (every two years)
  6. Athletes must have at least a 2.0 with Zero F's and no more then One N in Citizenship in the last official grading period (1st Q, 1 Semester, 3rd Q, or 2nd Semester)


The Gilroy High School Athletic Department has transitioned to an online registration process so registration is streamlined and more user-friendly (we hope!) 

A physical completed after June 1, 2015 will be required to complete your clearance. If you are having trouble getting a physical from your Primary Care Physician the We Care Health Center in Gilroy will take Gilroy High School students for $25. (408) 842-0278 appointment necessary.

The City of Gilroy runs an extensive aquatics program at both Gilroy and Christopher Pools during the summer. Please view the PDF from the City of Gilroy Activity Guide for information about rec swim, swim lessons, and pool parties.

Considering Playing a Sport in College?


Please make sure you are fully aware of all of the rules and regulations regarding participation in Division I or II athletics. You need to be certified by the NCAA Eligibility Center. Qualifications have changed!!!

Volunteer Drivers

  To volunteer as a driver you must:

  • Complete the GUS #153  (Driver information form and DMV pull notice)
  • Read and Sign GUS #155 (Instructions for Transporting GUSD Students)
  • Provide a copy of the first page of drivers Automobile Insurance Policy Declaration
  • Provide a copy of the Driver’s License
  • Obtain and Submit a DMV H6

Return all of this to the GHS Secretary
